Your First Appointment

Your initial evaluation will usually last about 1 hour. Your appointment will involve 1-on-1 time with your speech-language pathologist.

What should I bring?

Please bring the following items with you to your first appointment:

  • Insurance card

  • Form of identification

  • Method of payment (check, cash, or credit card) if you have a copay

  • Physician's referral if you have one

  • List of current medications (if not fully listed on the intake form)

  • List of any questions you have for your therapist

Should care partners and family members participate in the session?​

We are a family-centered practice and encourage care partner and family involvement in evaluation and treatment, especially for patients with communication or memory challenges. Participation levels vary based on patient needs.

Do you offer caregiver training?

For patients needing memory and communication support, both the patient and caregiver will participate in the evaluation. Follow-up sessions often include caregiver training, with patient involvement determined by the care partner and therapist based on your family's needs. Caregiver sessions are typically covered and billed through the patient’s insurance.

Cancellation Policy 

We aim to provide all of our patients the highest level of care. In order to accommodate the needs of our patients, we have decided to implement the following cancellation policy.

 

Please note that we offer reminder text messages, calls, or emails to help notify you of upcoming appointments.

 

This policy covers cancellations or no shows within 48 hours of your scheduled appointment time.

 

-$50 for the first two occurrences

-$65 for the third occurrence

 

New patients who cancel their first appointment within 48 hours of the scheduled time will be billed $65 late cancellation fee.

 

After three occurrences, you will be placed on "same day scheduling" list. This means that we are unable to schedule new appointments for you ahead of time, but we can call you if a day-of appointment becomes available.

 

No Show Policy

If you fail to appear for an appointment (no show) without providing the appropriate advance notification for 2 or more appointments within a plan of care, the office will reserve the right to cancel all pending appointments and to no longer offer services to you as a client.
